Cleveland Clinic MyChart Login

Step 2– Once the web page opens you will find the ‘sign in’ section at the top right. Provide your My Chart ID and Password in the white boxes and click on the orange button tagged “Sign In”.

Forgot ID
Forgot the My Chart ID? You can set a new one following the guidelines provided below.
Step 1– Go to the login homepage of Cleveland Clinic and tap on the link that reads “Forgot My Chart ID?” (For direct access to the recovery page, click here Link).
Step 2– In the next step, the user will be taken ahead of the recovery page. Provide the requested personal information and click “Submit”.

- Social Security number (Last 4 Digits)
- Date of Birth
- Zip Code
Step 3– Once your identity is confirmed you will be allowed to set a new password for your account.
